Forsyth Tech is committed to making everyone in our campus community safe, comfortable, and supported while they learn and work. We want to ensure that everyone remains safe and free from harassment, discrimination, retaliation, and violence. We depend on community members like you to identify and report any issues of concern so we can address them and provide impacted students and employees with support. Please report any concerning activity using the forms below.
Make a Report or File a Grievance
Read each description and utilize the form that best matches your report, complaint, or grievance.
Report sex-based harassment, stalking or dating, domestic, or relationship violence that is so pervasive, persistent, or severe that it would prevent a reasonable person from taking advantage of the educational or employment opportunities Forsyth Tech has to offer. This form is reviewed by the Title IX office.

This form is provided to encourage faculty, staff, students and other community members to report students
who exhibit behaviors of concern. Though the term “concern” is subjective, you may feel a student is exhibiting emotional or physical behaviors
that are of concern. This includes a non–instructional report when a student is jeopardizing the safety and security of faculty, staff and/or the student body. This form is reviewed by the Student Advocacy team and referred to the appropriate student support area.
Complete this report if you would like to file a grievance. The primary goal of this form is to produce a mutually acceptable solution for a grievance through an informal process. When necessary, a Grievance Advisor will move to a formal process.
File a GrievanceWhen a student believes that he or she has been treated unfairly by an employee of the College it is our intention to ensure that the student has clearly defined avenues of recourse such that the complaint can be resolved fairly and equitably. It is preferable that the complaint be resolved informally; however, when that is not feasible, this policy will ensure that a formal process for resolution is available.
Make a ReportThe purpose of the mandatory conduct referral is to provide assistance to faculty/staff that are experiencing conduct issues that are beyond classroom/campus management matters.
Classroom/campus management issues are to be addressed with faculty/staff’s department chair or dean. This form will be reviewed by the Conduct Office.
